Please complete the attached table of questions, and return these personal details (in confidence) so that we may determine whether our firm may assist you by checking for conflicts of interest and available expertise. We only retain the information you provide if you become our client. Otherwise, identifying personal information provided is deleted from our records in accordance with our Privacy Statement. Consultation fees are to be paid prior to consultation. Further fees and retainers will be discussed during your consultation. We accept certified cheque or cash, as well as Debit, Visa and Mastercard, however all electronic payments are subject to a $10.00 service charge. We will contact you if we are able to assist you.
